This is a ratings scale for children and young people to complete after interventions with healthcare professionals.  Developed by Scott D. Miller at the Child Outcomes Research Consortium.

There are separate ratings scales for children, very young children and older young people. For licensing reasons they can only be downloaded directly from the developer using the link below, but they are free and easy to download with no spam. They fit neatly with the Me first ethos.
