This guide has been developed to help young people from the age of 14 years with life-threatening conditions to know more about what to expect when they transition to adult services.

This guide has been divided into two sections. The first section talks about what you can expect at different stages of the transition process to adult services. The second section focuses on different aspects of your life that you may want to plan for as an adult. You are very much at the centre of it all.

This guide is produced and published by Together for Short Lives,  a UK wide charity that speaks out for all children and young people who are expected to have short lives.
