In healthcare, we use the word transition to describe the process of preparing, planning and moving from children’s to adult services.

Moving away from a team of doctors and nurses that a child or young person has been with for many years can be a daunting prospect. The University Hospital Southampton NHS Foundation Trust has developed the Ready Steady Go programme to support young people through this difficult time and to encourage them to get involved in the transition process to make them feel confident and happier about the change.
