Young Minds: Young people’s guide to transition


Audience:  young people, healthcare professionals, parents and carers

This guide explains what happens when young people receiving help from Child and Adolescent Mental Health Services (CAMHS) are reaching an age when they need to move on from CAMHS, and get the support that they need from adult services. This is often called ‘Transition’. This guide explains what should happen during Transition, and who should be involved in helping you get the support that you need.

There are three different versions: one for young people, one for parents and carers, and one for healthcare professionals.
