Conversations About Health Care Transition
What will it cover?
This is a four-hour course which includes:
- An introduction to the Me first Fundamentals of Communication
- Understanding of the experiences of CYP and their families as they transition and grow independence in heath care decisions.
- A framework of knowledge, that facilitates shared decision making and the developmentally appropriate, person-centred support of CYP during adolescence & emerging adulthood; before, during and after transfer of service provision (transition).
- A communication model which supports you to have conversations around transition. This can be applied holistically around a young persons life, encompassing specific areas such as developmental needs, learning disabilities and mental health.
- Skill-based practice using real life scenarios
- Identifying strengths in each professional’s current practice as well as opportunities for individual development and service improvement.
- Transition related resources that enhance CYP centred communication at a developmentally appropriate level.
Who is it for?
This course is suitable for any health and social care professional across both paediatric and adult services where CYP are transitioning. This includes those working in clinical specialities, complex physical disabilities, mental health and learning disabilities.
Why was this course developed?
Transferring from children’s to adult health and social care services can provide both risk and opportunity (Northumbria 2017). The Me First course Conversations About Transition training has been co-produced and is co-delivered by young people with lived experience of transition, to support the national call out to improve transition for young people in healthcare. Healthcare professionals, children young people and parents have supported its design and development through focus groups, alongside the Burdett Trust and Great Ormond Street Hospital . One of our key principles is involving young people in conversation about their treatment. It is paramount during transition we include young people by having the right conversations at the right time.
